If you are worried that someone may lack capacity to make a decision you will need to start by completing an assessment. The assessment needs to be completed on CareFirst 6 and there is also a Mental Capacity Act assessment form.
You can also use these simple assessment guidance notes to take you through the assessment process on CareFirst 6.
To help make your decision you can use the ehealthtracker tools. These will take you through a series of questions and checklists and give you a print out of the process. Before using the ehealthtracker for the first time, email the Deprivation of Liberty Safeguards team at firstname.lastname@example.org and ask to be added as a named user. You will then be sent a login code.
